Agrace Hospice & Palliative Care had implemented Epic as part of their Community Connect partnership. While the system was a tremendous boon for the hospice organization, there were opportunities for optimization to help Agrace serve the unique needs of its patient population, including challenges with scheduling, billing, and efficiency.
Nordic’s consultant partnered with Agrace on a pro bono basis to understand the organization’s vision for hospice and palliative care and
identify changes that would make the biggest impact. Nordic recommended a few key areas for optimization, including the use of one-click scheduling and work queues to make it easier for clinicians to triage and schedule patients. Nordic also trained Agrace clinicians on more efficient ways to use the system, helping providers see more patients and ensure each patient received the appropriate level of care.
